What Causes Nausea And Bloating After Fasting?
Please repeat endoscopy to assess acid reflux
Detailed Answer:
Hi XXXXXX,
Thanks for writing in to us.
I have read through your query in detail.
I am a radiologist and have served in a cancer institute for few years. A stomach cancer usually presents with the following symptoms
1. Nausea that is persistent and unexplained
2. Stomach pain
3. Vomiting that is persistent
4. Weight loss that is unintentional
5. Rarely bleeding from stomach in advanced disease
Other symptoms which can be seen are
1. Fatigue
2. Feeling bloated after eating
3. Feeling full after eating small amounts of food
4. Heartburn that is severe and persistent
5. Indigestion that is severe and unrelenting
However, fatigue, nausea bloating, heartburn and indigestion can also occur in acid reflux disease, which has been documented in endoscopy done 1.5 years back. I suppose that might have caused your symptoms, especially on the day following the fast.
In my experience, as you do not complain of loss of weight, persistent stomach pain or vomiting and the CT scan done 6 months back is normal then it is unlikely that you have a cancer.
In my opinion it is time to get another endoscopy done and look for any advancement of acid reflux disease or ulcer formation in the stomach and oesophagus.
In acid reflux disease, you need to take medications to decrease stomach acid formation as prescribed by your doctor. Other recommendations include having small snacks through out the day. Please do not go hungry for more than 4 hours in the daytime. Intake of citrus fruits and alcohol should be reduced.
Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.
Hope your query is answered.
Do write back if you have any doubts.

Hi XXXXXX,
Thanks for writing back with an update.
Though omeprazole is routinely prescribed to be taken 30 minutes before food to treat acid reflux, depending on the reflux, you may require to a higher dose or newer medication to keep the symptoms under control. It is important to re evaluate the acid reflux in your case since 1.5 years is an interval during which there might be some changes in the stomach lumen which needs assessment and your medications might need to be modified. At times there can be associated H. pylori infection which is aggravating your symptoms and this condition requires a specific multidrug treatment for about a week.
Right now assessment of the acid reflux is most important and it will show any recent changes in the stomach and this can only be known after doing a repeat endoscopy.
Hope your query is answered.
