I have gone through your question in detail and understand your concern.You are having cervical spondylitis with radiculopathy. There seems to be some nerve compression. It is confirmed by x ray cervical spine. MRI is helpful in seeing compression. Diacerine and glucosamine are helpful in delaying degenerative arthritis. Methylcobalamine is helpful in radiating pain and numbness. Analgesic such as ibuprofen pain relief. If pain is severe tramadol can give relief. You can discuss with your doctor about it.

As you are having severe pain in hand that comes up to shoulders I think it is due one of nerve being entrapment in degenerative changes (oesteophytes) i.e. it is neuropathy pain due to nerve compression at cervical spine . Second possibility is you are also having bilateral carpal tunnel syndrome along with compression at cervical spine called double compression syndrome. Third possibility in bilateral carpal tunnel syndrome without compression at cervical spine.
You can investigate it by nerve conduction tests of upper limbs.
You are already taking tegretol,gabantine and synthyroid. You can take tramadol if pain is not relieved by other analgesic.
You can get your thyroid status by T3,T4 and TSH as hypothyroidism are leads to swelling in subcutaneous tissue and carpal tunnel syndrome.
