While you have Stage 4 endometriosis and nose bleeds with bad headache during your periods ,is possible that you may have sinus endometriosis .
To determine that nose bleeding with headache is from endometriosis you may need to do a MRI(MAGNETIC RESONANCE IMAGES).

Hi dear again and I'm sorry for your issue.
All data you have reported reinforce the diagnosis that have suggested your doctor.
But the gold method to determine with 100% certanty, that ulcerous lesion is endometriosis is by doing a biopsy or histopathology of lesion.
But while your doctor have ordered you to make an OPG it can be done at a very low price compared to the CT/MRI and is very sensitive for gross localization of Nasal Cavity/Ethomodial sinus/Maxillary sinus lesions & other pathology ,as sinus endometriosis.
In my opinion you should contact and with your follow-up gynecologist to discuss
for a long treatment of your spread endometriosis.

This is obviously a rare situation??
If the ulcer is endometriosis what would be the treatment options?
Failing hormonal treatment what can be done to stop the spread of endometriosis? I am just worried about where else it might turn up.
I have scheduled a follow up with my gynaecologist in 2 weeks.

If the mouth ulcer is endometriosis than the treatment is the same as per pelvic endometriosis ,because it's clinic vary form hormonal changes in your body during your periods.
The choice of treatment in endometriosis depends on several factors such as:
Woman’s age(you are a young women)
Severity of symptoms(your symptoms are really bad)
Fertility status(I don't know)
Stage of the disease(is stage 4)
Prior treatments (if any) and treatment response and side effects (if any)
Until hormonal treatment have failed, risks and benefits of different treatment options need to be thoroughly discussed and considered prior to treatment selection.
What kind of hormonal treatment did you take ?

I'm sorry that both hormonal and surgery therapy have failed .
The best treatment of this stage of endometriosis may be is :
"having a hysterectomy or salpingo-oophorectomy ".
But having this procedure done vary from your parity(how many children do you have ) and does not guarantee that the lesions will not return or that the pain will go away.
Endometriosis symptoms and lesions may come back in as many as 15% of women who have a total hysterectomy with bilateral salpingo-oophorectomy.
Hence wait until you will contact will your gynecologist and he /she will give any benefit solution for your health issue.
