Question: Hi there,
I have a problem that I have asked several questions about and just wanted another opinion before I go back to GP.
I have had an uncomfortable feeling in my groin area (it feels like it is on the panty line) has been there now noticeably for a couple of months. It feels like there is something there,only on left hand side, cannot be seen by the eye or by touch. I have been to the GP who has take urine and swabs, done an internal and checked for lumps or swollen glands. The GP felt or saw nothing that would raise red flags. Great but that does not solve the mystery. I feel it when I walk, it started as though it were a shaving rash or ingrown hair, just irritated, it is now quite uncomfortable I have tried to wear loose underwear but I can still feel it. As there is nothing on the surface, could it be inside, I was wondering if it could be a slight prolapse or hernia that cannot be seen, I do get very constipated and strain going to the toilet, is then anything you can think of that would not present itself to the eye and should I request an ultra sound ? I dont want to leave it as it is quite uncomfortable. Any ideas? I have asked a couple of docs on this site and they have given various different suggestions, one said it could be bacterial but there is nothing to see? Baffled!!! Thank you

I have gone through your query with diligence and would like you to know that I am here to help. I completely understand how you feel and would like to put forward my observation and a list to help better understand your situation:

1. Bacterial infection: Unlikely as it should have shown up on a urine and swab examination, as well as the naked eye

2. Ingrown hair or folliculitis: Commonly seen in the groin region due to the constant friction and from actions like shaving or hair removal. Unlikely here because it is clearly seen by the naked eye

3. Hernia: Is likely, although most of them present through a physical examination which I am guessing your doctor has conducted, but some can be masked and so I would agree with requesting for an ultrasound examination, so please do request for it

4. Hematoma (collection of blood): Most of them present with physical signs, but a physical examination usually helps diagnose it. But the ultrasound will also help to diagnose or rule this possibility

5. Viral infections: Presentation clearly seen through the naked eye

6. Sexually transmitted infections: Presentation clearly seen through the naked eye

7. Surgical procedures: I see in your history that one of your ovaries was removed, could you mention which side it was removed from?

8. Fungal infection: Usually seen through a physical examination, but some times can be skipped and so I would recommend the use of a topical anti-fungal cream (e.g. clotrimazole, miconazole, ketoconazole, etc.)

9. Friction: If you do not mind experimenting, then please do try to find differently shaped inner wear, which reduces friction with the region you have been having trouble, and also try to start using a talcum powder in the creases of the groin which helps to reduce friction. If this helps, then great; if not, then it still did not really cause us any harm

From the above list that I have come up with:

+ 3 and 4 require an ultrasound examination of your abdomen and pelvis
+ 7 still needs some discussion and thought
+ 8 may have to be considered until the treatment fails to help improve your symptoms
+ 9 can be tried if you do not mind

Thank you very much for your detailed reply. To be honest it was some years ago and I am not entirely sure which ovary it was without checking with the GP, I only found out there was a problem as the tube was twisting causing extreme pain, it was found under ultrasound. Could something like that affect me after so long and if so should I be concerned? I have tried different underware but I can feel it even with nothing on. I guess ultrasound is the best way to put my mind at rest.
