What Causes On And Off Sharp Pain In Genital Area?

Question: Hello. I have a sharp pain in my genital area off and on every other day. I haven't had it for about 5 days now. About 2 months ago, a bump appeared on the tip of my penis and now it's finally almost fully gone away. My primary care physician and also urologist both told me that it doesn't appear to be an sti. Now I developed a red rash on an area of my testicles. It stings frequently without me even touching it. Been there about 2 days. My penile skin is sometimes extremely sensitive as well. I don't feel sick or anything. No swollen lymph nodes or fever. I'm just trying to get an answer to what could be going in. I feel like it's a bacterial infection but no one that I see will treat me for one. I'm just going around in circles here

I have gone through your query with diligence and would like you to know that I am here to help. I do agree with your doctor that this does not sound like a sexually transmitted infection, without looking at it although I cannot be making such a statement.

But from the given description though, I do strongly suspect some underlying cause but cannot be sure without further investigations. But for now, I shall put forward a list of conditions I am suspecting and would like you to take up with your doctor to rule out and narrow down the probable cause to:

+Yeast/fungal infection: Yeast grows and thrives in moist and warm environments, this is especially accessible in the genital region especially when the area is not kept clean and dry. So if at all this is the case, I would suggest you try to keep the region clean and dry and request your doctor to conduct a simple urine analysis, urine culture and sensitivity examination and even a skin biopsy if possible

+Dermatitis: Is also possible in moist and warm environments, and can especially be caused due to extreme temperatures

+Bacterial infections: As you suspected, does certainly exist on our list of possible conditions

I would thus request you to just visit a clinic or a hospital and request for the following investigations:

+Urine analysis
+Urine culture and sensitivity examination
+Skin biopsy of the region (may not be easy to get without a doctor's order)

I do suspect some abnormality and would like to help you further, but I hope you understand that I cannot provide you guidance on treatment unless a diagnosis is established. I hope and pray you are able to take this up with your doctor or at least get the required investigations conducted and write back to me, as I would be more than happy to interpret them for you.

Follow up: Dr. Shoaib Khan (2 days later)
Hey thanks for your reply Dr Khan! About a month ago, I had a urinalysis and it came back negative. The rash developed into about 6 open bumps or ulcers as I think most doctors address them. I have an appointment with my PCM tomorrow to have it looked at. Again, he asked me the same questions like "Are they oozing fluid? Are they blisters? Did they start as blisters and burst open." I told him no and he immediately ruled out an STI again over the phone. He also asked if I had a fever or any other abnormalities. So hopefully he can look at it tomorrow and tell me whats going on. I have been sleeping kind of weird though. I sleep on my sides and lately I will wake up with tender skin in the area that the "ulcers/rash" developed. So maybe it could be from the way my scrotum skin pinches between my thigh. But who knows. I'm just praying and hoping it isn't herpes. I keep getting reassurance that it isn't but there's still that one bug in my ear. Thanks for the help. I know you can't diagnose me without seeing anything. Your response was very helpful. Do you have any more nuggets of advice?

Thank you for all the information you have provided and the understanding you have shown throughout.

I know I may not be able to diagnose you well enough without physically examining you or even without ordering and reading a few supporting laboratory investigations, but I would certainly like to add to our discussion the fact that 'ulcers' as your doctor puts it or 'open sores' are indeed in majority of cases linked to sexually transmitted infections.

The list includes herpes, syphilis, mononucleosis, chancroid and in few cases even HIV (due to an immunocompromised state). Please make sure your doctor goes about all the investigations to rule out all these conditions whether the appearance seems to indicate them or not. I would certainly like to do so and not depend on physical presentation which at times (although rarely) can be deceiving.
