I have gone through your query with diligence and would like you to know that I am here to help. I do agree with your doctor that this does not sound like a sexually transmitted infection, without looking at it although I cannot be making such a statement.
But from the given description though, I do strongly suspect some underlying cause but cannot be sure without further investigations. But for now, I shall put forward a list of conditions I am suspecting and would like you to take up with your doctor to rule out and narrow down the probable cause to:
+Yeast/fungal infection: Yeast grows and thrives in moist and warm environments, this is especially accessible in the genital region especially when the area is not kept clean and dry. So if at all this is the case, I would suggest you try to keep the region clean and dry and request your doctor to conduct a simple urine analysis, urine culture and sensitivity examination and even a skin biopsy if possible
+Dermatitis: Is also possible in moist and warm environments, and can especially be caused due to extreme temperatures
+Bacterial infections: As you suspected, does certainly exist on our list of possible conditions
I would thus request you to just visit a clinic or a hospital and request for the following investigations:
+Urine analysis
+Urine culture and sensitivity examination
+Skin biopsy of the region (may not be easy to get without a doctor's order)
I do suspect some abnormality and would like to help you further, but I hope you understand that I cannot provide you guidance on treatment unless a diagnosis is established. Hello once again sir.
Thank you for all the information you have provided and the understanding you have shown throughout.
I know I may not be able to diagnose you well enough without physically examining you or even without ordering and reading a few supporting laboratory investigations, but I would certainly like to add to our discussion the fact that 'ulcers' as your doctor puts it or 'open sores' are indeed in majority of cases linked to sexually transmitted infections.
The list includes herpes, syphilis, mononucleosis, chancroid and in few cases even HIV (due to an immunocompromised state). Please make sure your doctor goes about all the investigations to rule out all these conditions whether the appearance seems to indicate them or not. I would certainly like to do so and not depend on physical presentation which at times (although rarely) can be deceiving.
