Brief Answer:
Anovate is unlikely to be helpful

Detailed Answer:
Hi
Welcome to Healthcaremagic
Greetings of the day

Dear Ms

Persistent pain in cause of anal fissure is likely to be due to

1. Underlying constipation and passage of hard stools

2. Associated spasm of muscle around the anus

As fissurectomy and sphincterotomy have already been done and also Diltiazem ( calcium channel blocker ) which relaxes the muscle has been tried, anovate is unlikely to be helpful.

I would suggest you the following

1. Ensure good intake of water : 2 to 3 liter a day. Water intake is very important for ensuring formation of soft stools. As any hard stools will further worsen fissure

2. High fiber diet : 2 servings of vegetable salad and 2 servings of fruits especially citrus fruit will ensure adequate intake of fiber in diet.

3. Hot Sitz bath : To a tub of lukewarm water add 2 spoons of antiseptic solution. Sit in it for 15 to 20 minutes each session twice a day. The warmthness will reduce pain and relax the muscle

4. Laxatives like Lactulose will ensure soft stools

5. Continue the medicine as advised by your physician

If the pain still persist then do consult your physician for thorough reassessment.

Wishing you and your family a very happy and healthy life.

Take care

Answered by Dr. Deepak Kishore Kaltari (14 minutes later)
Brief Answer:
Lactulose can be safely used in Diabetic

Detailed Answer:
Dear Ms

Lactulose even though its a disaccharide (semisynthetic disaccharide of fructose and lactose), it does not get absorbed from intestine nor digested hence acts as Osmotic laxative. So it does not affect Diabetes. It can be safely be used and there is no cause for concern.
