Brief Answer:
Most probably costochondritis.

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.
To recapitulate: Female/83 - Cholecystectomy in 2004 - fatty liver and a large cyst as told by Radiologist - have had pain and feeling like something is pressing on right side, under the breast, feeling like a lump - like a belt or something pressing on upper abdomen - had abdominal CT scan about a year - says everything is fine and probable a muscle...

From the history I can think of the following things:
This can be due to costocondritis, wherein one gets inflammation of the lower floating ribs, the inflammation and internal edema spread to the muscles attached to it and here these are the muscles of the upper abdomen that originate from the lower ribs.
Fatty liver per say does not cause such a thing.
Large cyst can cause heaviness but not the pains unless there is sudden increase in size like due to internal bleeding or so.

I hope this answer helps you.
