What Causes Pain And Pressure In Left Side Of Pelvic Area?
Question: Hey just curious. I had a transvaginal ultrasound two weeks ago and right after had pain in pressure suddenly on the left side of my pelvic area where I never had any pain or pressure before, ever!!! The cyst was on my right ovary and since the transvaginal ultrasound, its like she tweaked a muscle or something on my left groin area but when I walk now I get cramping feeling in my lower left side (never before the ultrasound)
It feels like pulled muscle some days, burning sensation in my groin area on left or it moves up along left side seems like my desending colon area or something with cramp feeling.
Any idea what is happening and if so how long before it might go away?
Could it be a muscle or nerve that got tweaked? I cant believe this is happening, went to see how right ovary was doing with cyst and now I have pain pressure on otherside and it happened like 30 seconds after it ended started feeling pressure.
And to top it off the cyst has resolved.
Thoughts?

Brief Answer:
it could be a tweaked nerve
Detailed Answer:
Hello,
I have read your query and I am sorry about your current symptoms.
The pain and the pressure that you are feeling on the left side of your pelvic area is probably because of a tweaked nerve, it could be the pudendal nerve or other nerves surrounding the area.
It might need a maximum of 3 weeks to go away. If it does not subside by then, you must get back to your doctor and discuss about the same.
Whatever be it, wait until your periods. Once you menstruate, the pain and pressure should ideally subside.
I am glad the cyst on right side has resolved.
Get well soon.

Thank you for your reply.
It happened 2 weeks ago and I got my period 6 days ago so it is just ending but didnt help the situation.
It's weird, its like the nerve is attached to my descending colon. The day after it happened when I went to poop, I felt a pinch in my lower left pelvic area which ever happened before the transvaginal doppler ultrasound they did the night before.
Now when I walk after a couple of minutes I get pressure in my descending colon area, like a cramp. Maybe the nerve you mentioned attaches to that area or maybe its some kind of referal pain?
Its in my lower left groin and comes up along that angle up to around my left pelvic bone.
Also sometimes I have had a nerve pain shoot down the inside of my left leg down to my ankle.
Do you really think it will work itself out or would I need to go for physical theropy?
I dont have any pee issues (which I saw when I looked this situation up on the internet a lot of time this kind of thing makes someone feel like they have to pee alot but they really dont, its just nerves or muscle spasms down there)
I dont want to have a PT start touching down there and set something else off but also right now I cant walk more than a block without getting pressure cramping feeling on left descending colon area and it gets pressure in lower pelvic area between bone, groin and leg when i sit up for about a minute so I have been only standing or laying down (what kind of life is this)
I have to go back to work and dont know what to do?
Any other thoughts or suggestions?
Could it be some kind of muscle that goes along groin up to pelvic bone lower descending colon area or if that usually a nerve?
Is this common after transvaginal ultrasounds?
Again, any thoughts you could give on this would be so very much appreciated!!!
Thank you again!!! XXXXXXX

Brief Answer:
get transabdominal ultrasound done
Detailed Answer:
Hello XXXXXXX
Thanks for writing back, it's my pleasure to help you.
Now, that the pain seems lot more obvious, I would suggest that you get a transabdominal ultrasound scan without disturbing the pelvis from inside (this happens through transvaginal ultrasound).
When we do a transabdominal ultrasound, we could look for problem in the descending colon, and also any muscle that could be strained.
If the transabdominal ultrasound is normal, then you could possibly try physical therapy.
Do you feel any tenderness in the muscle in the abdomen or groin area? If so, then it could be muscle sprain. However, a tweaked nerve seems more of a possibility in your case.
Drink plenty of water, and before the pain makes you debilitating, get a transabdominal ultrasound done.
Get well soon.

Thank you again for your response.
Just a few more questions:
I am going back to see my doctor next week. My insurance will cover an ultrasound and/or an MRI no problem. I dont need pre-authorization with the type of health insurance I have. Which would be better to see if it is a tweaked nerve or muscle spasm or strain or sprain, MRI or Ultrasound? What is the difference between the two? What would the Ultrasound show vs what the MRI could show?
If I did do MRI, would I need contrast? Is that what would show things lighting up in the picture if something is tweaked or injured? Would it light up if I have a tweaked nerve or muscle in my groin, pelvic descending colon area?
If I just wait a few more weeks, do you think it will just reslove on its own? For example if it is a tweaked nerve, can anything be done about it regardless or does it just have to heal on its own with time?
As always, thank you so much for your advice. It is so very much appreciated!!! XXXXXXX

Brief Answer:
ultrasound would suffice
Detailed Answer:
Hi XXXXXXX
A transabdominal ultrasound scan of the whole abdomen and the pelvis would suffice to see for tweaked nerve or muscle spasm or strain or sprain. Only if need be, a MRI would be done. As such, in your case, it is not required.
Ultrasound looks at the organs as a whole while MRI takes serious slices or cut sections. MRI would reveal minute details which is not required for you. MRI studies in depth of all the organs like looking for very tiny masses etc.
MRI would show spillage, swellings, minor changes in the size or shape of the organ. It goes well within as organ as well, while ultrasound takes the gross pictures of the organs.
Yes, if you wait for a few more weeks, this should resolve on its own. Just give it some time, perhaps 4 weeks, it should go away soon.
I hope your doubts are cleared. My pleasure to help you.
Take care!

You have been so very helpful!!!:)
As a last request, there is one other problem I am having down there and you seem very smart and knowledgeable so I will ask your thoughts on this last topic.
All this started 6 weeks ago when I went for my annual gyno exam and when she did the pelvic exam, I suddenly got horrible pain on my lower right side. She said she might of tweaked my right ovary and it should clear up in an hour or so not to worry.
Well, an hour later it was 100x worse!!!
I went back and they did what was the first transvaginal ultrasound (that one did not bother me after at all) and found I had a 3cm partially ruptured cyst on my right ovary.
That is what the pain was from. It was leaking fluid into my abdominal cavity.
About 3 days later the pain calmed down but I have not been able to sit up for more than 10 minutes without getting really bad pain and pressure on my right side where the right ovary is where she partially ruptured the cyst.
I was going to work by car and just suffering through the pain and pressure i would get from driving, and then I would laydown in my car for a half hour before going into to work so the pain would subside.
I have been working at my job standing up with my computer elevated on a higher desk.
Standing and laying down I get no pain on the right ovary where the cyst was, I just get pain and pressure when I sit.
So two weeks ago I went back for a second transvaginal ultrasound to see what was going on with the cyst and they said the cyst on the right was gone, must have fully ruptured after my last sono and the left had none so I was all good cyst free.
I told her the right still hurts when i sit like the cyst is still there. Maybe when she ruptured the cyst on my ovary with her finger she toar part of it off my ovary and my ovary is still trying to heal?
She said it should be better in a few weeks and told me to come back next week if it is still bothering me and she will do another pelvic exam.
Is she kidding me?
Like I would let her go blindly in there again after what she did to me the last time? Especially if my ovary or something around it that must have gotten injured that day when she toar the cyst which is now obviously trying to heal? What good would it do for her to blindly push on that with her fingers again?
The scary thing is when she did the pelvic exam she didnt even realize she hit a cyst on me. She said everything feels good see you next year when she was done.
My last question to you is, I still have the same pain/pressure on the right where the cyst was even though the cyst is gone. And now pain additional issue on the left since the vaginal ultrasound 2 weeks ago which is what I originally wrote you about.
For both issues, the ovary on the right still hurting when I put pressure on it from sitting up and the nerve, muscle issue on the left since transvaginal ultra sound which the tech said she didnt go any higher in me than my cervex, what would a pelvic exam do or show that another ultrasound couldnt do or show?
Is there any reason to let her go in there again and feel around? She mentioned the tweaked feeling I have with the cramping on the left side since the ultrasound, when I was getting it done they hit up against the cervex which moves the uterous and everything around a little. Could something have moved and gotten stuck or in the wrong place on the left and that is why I am getting the cramping? An ultrasound could show if something was out of place correct? She doesnt need to do a pelvic exam we could do an ultrasound instead yes?
She gets very demanding that she needs to do pelvic exam next time but unless you can tell me a reason that would be perfered to an ultrasound to see if anything is out of place or whatever, do you agree that ultrasound woudl be the way to go, especially since the right ovary still hurts since she ruptured the cyst on it with her finger and might be healing and her hitting it again could make it worse?
Also, is it normal to still have pain on the right ovary 6 weeks after the incident happened where she hit the cyst and ruptured it? Could the ovary just be healing still or something around the ovary?
Thank you again for taking the time to listen to my story. You caring and compassion are very much appreciated!!! You are helping my well being more than you know!!!

Brief Answer:
ultrasound is preferred to pelvic exam
Detailed Answer:
Hi XXXXXXX
Thanks for writing back. Now the picture seems to be much clear.
Let's rewind and go back.
It seems like the doctor did mess up with your first pelvic exam. She did tweak your right ovary, going by your description. Also, she cannot leave it that way saying it should clear up in an hour or so and not to worry. That is definitely not a good doctor should be doing. A 3cm partially ruptured cyst on right ovary would cause tremendous pain to the patient that could be unbearable. It is a medical emergency, and needs to be treated as In-patient, especially so when someone is in terrible pain.
I empathize with the amount of pain that you went through; it must have been difficult for you.
It is probable that the ruptured cyst and the right ovary is still trying to heal.
The doctor is probably kidding you :( It is better to not go for a second pelvic exam or ask the higher experienced doctor to carefully do a pelvic exam on you. It probably would do no good for her to blindly push on that with her fingers again and definitely not do any much good to you. In fact, it could possibly increase your pain. Avoid it as much as possible. A transabdominal scan would be less painful.
A pelvic exam would only elicit tenderness and cause more pain to you. The ultrasound scan would hurt you less and we could see thing better than a pelvic exam.
There is no reason to let her go in there again and feel around. It is possible that the ultrasound probe would have caused trivial trauma to the cervix and the surrounding structures but that does not cause pain of this much severity. It is most likely that the ruptured cyst on the right side is still spilling fluid into the abdomen, that is passing on to the left side and the surrounding pelvic areas, and unfortunately causing your symptoms.
An ultrasound would definitely show if something is out of place. There is no need to do a pelvic exam for the same.
In all possibilities, try and avoid the pelvic exam and opt for an ultrasound instead to remain relatively pain free, and also ultrasound is less invasive and painful compared to pelvic exam. A pelvic exam inevitably warrants for a ultrasound whatever be the result of it. So, why not straight away to the ultrasound instead?
Discuss with your doctor about this.
No, it is not normal to have the pain on the right ovary 6 weeks after the incident happened where she hit the cyst and ruptured it. I doubt the ovary is healing, perhaps there is still some spillage happening that is causing the continued pain.
Towards the end, please note my points above and come to a wise decision. Discuss logically with your doctor. I hope she understands.
I wish you quick recovery, XXXXXXX

Thank you again for all of your help.
I tried to send you one last response but I some how lost it so here is the short short version.
My last question to you (I promise this is the last) is the ultrasound report (I have a copy) from the last one i did 2 weeks ago says that there is no more cyst and that there is no pelvic free fluid so the pain I still get in my right side by my ovary since she originally hit the cyst there 6 weeks ago (pain happens when I sit up for more than 10 minutes, pressure/burning type feeling starts and then only goes away if i lay down for an hour or so) do you think that is ovary trying to heal from the trama of the cyst she partially ruptured (maybe she hurt my ovary when she hit it, maybe it toar it or something?)
I am just wondering if there is any kind of scan that could show detail if there was a cut or tear on my ovary that is healing or is the only way to see if they did laporscopic surgery to go in and look?
The pain seems to be getting a bit less each week but maybe when i sit up and it bothers me I am re-injuring it and that is why it cant heal?
Perhaps I need to take a few weeks off from work and just lay on the couch to let it properly heal or should it be able to heal regardless of my sitting up sometimes and irritating it?
That would be my last question to you, what you think is going on with the fact i still get pain/burning feeling (not at all as bad as when it first happened) when i sit up where the cyst used to be but is now totally ruptured and gone, and no fluid showing anymore either? What is causing the pain? Injury to the ovary that needs to heal? Does that just take time? Am I re-injuring when I sit up and that is why it cant heal and i need to just take time off and lay still for it to finally heal right and if so, any idea how long that could be? (2 weeks, 4 weeks, 6 weeks?)
That is the last of my questions and I promise no more after this!!!
Thanks again for everything and I will be going to see a different gyno in 2 weeks that a friend recommended for a second opinion as well, and no pelvic exams for a really long time;)
God bless you and your family as well:) XXXXXXX

Brief Answer:
give it time to heal
Detailed Answer:
Hi XXXXXXX
Yes, this could be the ovary that is trying to heal from the trauma of the cyst that was ruptured.
Ultrasound would be the best investigation here. Laparoscopy can be used for both seeing the ovaries, and management or surgical drainage of cyst, if any. Hence, laparoscopy is both investigative and therapeutic to the patient.
I do not think you are re-injuring it. Try not to sit continuously for long hours. It would be better for you to take a few weeks off from your work and lay on your couch to help in faster and complete healing.
There could be residual fluid from the cyst that did not show up on the scan, and that is probably causing your symptoms.
As I said earlier, give it some time and adequate rest.
Good luck with your next gyno doctor's appointment.
Meanwhile, drink plenty of fluids orally. It is always good.
Take rest and take care!
