What Causes Pain And Redness On Abdomen Post Cesarean Delivery?

Question: I had undergone a cesarean delivery on 17:11:14. After that from 3rd day she is feeling a constant pain in her abdomen, coupled. Earlier, when contacted to consultant gynaecologist, she said its normal and part of healing. But the problem persisted and grew further, coupled with swelling and redness of marks on abdomen. The problem effected the daily routine of my wife, even so she could not sit, and move without help.We done MRI scan on 12:01:15 which revealed "edema on scar site/ normal bowel movement".A treatment was run for a week at hospital including antibiotic dose of pipzo, amikacin through iv, which did give the relax, but the problem again resurfaces after one week.We searched on internet about the problem and got information about abrasions during surgery, which requires another surgery to treat it.

Hello

I have gone through your wife's findings in detail.
Her problem may be due to persistent infection at the stitch site.Ultrasound of stitch/scar area are can be done to assess any collection at the stitch site.It can also assess scar thickness.If there is any collection then it can be drained out and can be sent for culture and sensitivity.

Adhesion is also one of the possibility but presentation is early in her case.Adhesions are due to formation of fibrous bands in response to operation. But her problem is continued from 3rd day of operation,this is against adhesion.

It is good that MRI findings are almost normal except edema at the stitch site which is manageable.

Stitch opened form inside is unlikely.Suture used internally are very strong and these are absorbed in few days.
There are chances of bacterial growth at the local site.
Reappearance of symptoms may be due to persistent infection.You may need specific treatment for those bacteria.
Rapid heart beat,chest pain etc may be due to anxiety but is important to exclude cardiopulmonary causes through proper investigations.
I don't think you have serious findings.

Hello

Tract formation may be due to infective etiology but tubercular cause should be confirmed by further investigation.It may be due to other bacteria also.
She need clinical correlation and tests for confirmation of tuberculosis.
CT scan of abdomen can be done after evaluation.

Sir got another usg tday as doc wasnt satisfied with the findigs..attaching new rport pls guide..what is this report indicating

Will mri be helpful for furthr evaluation as ct contains a lot of radoations nd dnt wanna go for it

Hello

As we discussed in first query,recent findings indicate small amount of collection at operated site.
This findings seems to be more reliable.I have also encountered so many similar cases and mostly it is treated conservatively.

Earlier report regarding sudden appearance of tuberculous tract after operation seems to be unconvincing.

If there is strong doubt of collection then neither CT scan or MRI is needed.This is a manageable condition.

I have got that stool test done reports are still awaited..The surgeon whom I was consulting has prescribed me a chest x ray and tb gold test..x ray chest came normal and tb gold test was also negative.got the mri done today ..I m enclosing the teports received so far..pls guide..I went to my gynae also.she has prescibed me sumag ointment twice a day

Had it been a necrosis wouldnt that come in mri??

Am I really having an infection that is fatal ((whatever is this fluid collection or necrosis is??))as every doc has its own saying??

Latest MRI report is relieving for us.There is only minimal soft tissue tract at the operated site.There is no collection at the operated site.This shows improvement.

You are responding well to treatment.You should continue it.You will recover soon.
You can continue medicines for asthma and medicines prescribed by your gynecologist.Sumag ointment will further reduce inflammation and it is very good for local application.

There is no necrosis etc commented in your report.
Mild adhesions are normal post-op findings.

It is good that T.B tests are negative.
