Thanks for the query.
pain in any part of body making you uncomfortable bounds to cause concern , specially when you are in sixties,for early diagnosis & prompt management forms basis of health in advancing age. I understand your concern.
Back pain can be from anything that builds up back structure. Like muscles, tendons, ligaments, bones( spine )& disc problems, Renal problems.. But from the characters you describe.. I would think of following causes for it in your case.
-Back sprain & strain.. which involves extensive physical stress with muscular twisting, stretching.Here the area gets sore & painful on touch. & needs rest/medicines to relieve pain.
- Osteoporosis..follows calcium & vitamin D deficiency.. bones become thin & fragile.. can not bear stress .. leading to pain & soreness & some times fracture of bones.
-Protruding spinal discs( due to degenarative changes ) can cause such pain .. mainly is restricted to specific part of involved spine.
- Pyelonephritis.. this is kidney infection. Usually pain starts suddenly behind the lower ribs,& travels to lower back & front to groin..Here urine is cloudy,foul smelling,May be associated with fever with chills & pain in urination too.
Kidney stones can also present with the pain like you have.
* In this context I would advise you to consult your family physician . He will examine you in detail, advise few investigations & treat you specifically OR he may refer you to Urologist/ orthopedic surgeon/ general physician for further evaluation.
* Investigations done will include- Blood test ( Calcium. vitamin D levels)/ Urine test routine, culture & sensitivity./ X ray, MRI,CT scan as per need to diagnose spinal involvement/ urinary calculi )
* Management would be cause specific ( calcium/ vitamin D supplements , antibiotics,NSAID drugs, physiotherapy, surgery as per cause )
Home care- Take rest. You can use hot/ cold compresses. apply muscle relaxant ointment & take oral pain medicine over the counter. DO not massage
