Brief Answer:
need to see doctor

Detailed Answer:
Hello,
It sounds like you have either orchitis (infection in the testicle) or epididymitis (infection in the epididymus). These infections can be very painful and you should see a doctor for an exam and antibiotics. I see that you have tried the levofloxacin but that may not be the best antibiotic for the infection (if you have one).
If the pain is severe there is another even more serious condition called testicular torsion where the testicle gets twisted around and the blood supply gets cut off. This is very serious so if the pain is very severe you need to go to emergency now.
If the pain is not severe, I still think you should see a doctor today since if you need antibiotics this will only get worse the longer it goes untreated.
If you cannot see your doctor today I suggest that you go to an urgent care or to Emergency for evaluation and treatment.
I hope you are better soon,
Dr. Robinson
