As far as The pain and swelling is concerned, it can be because of the infection in the tooth which has been filled 20 years back. This can occur because of the chronic irritation by the filling material to the pulp or secondary decay below the filling.
The swelling is because of the formation of pus secondary to tooth infection which has spread to the surrounding soft tissues.
Consult a oral physician and get it evaluated. You need to take a radiograph to confirm the diagnosis. Later the tooth can be treated with root canal treatment.
For the relief of pain and swelling you can take a course of antibiotics like amoxycillin and metronidazole (if you are not allergic). Along with this you need to take an analgesic like ketorolac tablet (one tablet) for immediate pain relief. You can disperse this tablet and drink. It reduces pain in 20 minutes. ( if you are not allergic or sensitive to this drug).
It takes 1-2 days for the swelling to come down.
