What Causes Pain And Tightness At Incision Site?

Question: Hello doctor,
Just wanted to inform you about the recent finding.
Two days ago I noticed the the pain and tightness which I feel is not on the area where the incision was done. Hence I do not have pain in the area where I thought the scar was formed.
I went for a checkup with the the doctor who did my surgery and pointed him the area where I have pain. Although again he did not see or feel anything abnormal but he felt that the area I am pointing is external sphincter muscle.
He has prescribed me Diltigesic gel for 5 days and after that if I do not feel improvement he has told me to take Gabapin M tab for 5 days and the consult him again.
I feel pain and swelling on the area which the doctor is referring as external sphincter muscle.

Hi.
Noted all the recent findings you have mentioned, the Surgeon found out and told you.
With the fresh findings it is a good thing that the scar is well formed and not the cause of any pain or tenderness.
The pain is at a different place which the Surgeon says is external sphincter muscle.
It is possible that the internal as well as internal sphincter muscles might be in spasm wherein Diltigesic helps by increasing local blood flow and relaxes sphincter hence worth trying. This alone is tried to see its effects.
Gabapin M works by reducing or controlling the neurogenic pain. Hence to be tried after 5 days of Diltigesic gel application, if Diltigesic does not help.
This will help therapeutically as well as for diagnostic purpose.
Hence best to avoid steroids at the moment as they may not help.

Please do this and let me know the results.

I can feel swelling and pain ata particular point which feels like a small scar shape. That is why i got confused

Is a pain and swelling like this possible due to sphinster spasm.

Yes, it can be spasm or a combination of spasm and fibrosis, sort of a fibromyalgia.
Fortunately, this may not be an abscess as by this time it would have burst and opened up.
