Periodic dull aching / throbbing pain in the right hypochondriac / below ribs.
The most possible cause could be the gall bladder pathology ( gall stones, inflammation of the gall bladder, sludge in gall bladder), followed by the liver pathology and biliary obstruction.
Next possible cause - long standing renal calculus / stones with mild obstruction may cause the dull periodic radiating pain to the right upper quadrant.
Other possible cause could be gastric ( peptic ulcers) and finally musculoskeletal pain.
The best way is get an ultrasound scan in an empty stomach, which will help to evaluate all the possible reasons in the abdomen. Depending on the findings one can plan the treatment.
