Question: Hi Doctor,
I noticed today a lump in between my anus and my scrotum. When i feel with my finger it feels slightly swollen. Its closer to the anus. What could it be? I did have very spicy food yesterday and my last stool was a little painful, could it just be inflammed? Thanks.

The picture shows the perineal area inflamed, skin involved, redness and on correlation of you history looks to be a skin infection with cellulitis.
Anal fissure is not seen.
I would suggest you the following in such a situation:
Clean the area with plain water, let it dry up or make it dry and apply solution povidone iodine to make the skin sterile.
getting started with proper antibiotic and anti-inflammatory medication as early as possible may abort the attack from progressing to a larger cellulitis and abscess as you have noticed the problems today. You need a prescription for this and hence need to see your Doctor.
