Brief Answer:
possibly related to the soft tissue

Detailed Answer:
HI, thanks for using healthcare magic

It is possible since no bony injury or changes are seen on x ray that the the pain may be due to a problem with the soft tissue such as the tendons or muscles in the ankles

(1)Injury or inflammation of the achilles tendon can cause pain which is worse on walking up stairs or hills.

This can be diagnosed by examining the ankles, there may be swelling or thickening.

Treatment involves(a) rest from any aggravating activitities,
(b) use of anti inflammatory pain killers such as ibuprofen/cataflam or naproxen,
(c) exercises such as the calf stretch, heel drop
(d)physiotherapy may be needed
(e)some persons require steroid injections

It can sometimes take months for improvement

(2)Another possible cause is plantar fasciitis- this usually causes pain by the heels

Treatment involves rest, the use of the medication mentioned along with exercises as well

You may want to consider seeing an orthopedic specialist for assessment

I hope this helps, feel free to ask any other questions
