Question: 18 year old with breast pain in right breast? Lumpy but she is very small with no fat in breast

Hi.
Thanks for your query for a girl of 18. Understood that she has breast pain in he right breast. Lumpy but she is very small with no fat in breast.

The probable cause of painful lump in the breast irrespective of the fat and size at the age of 18 is most probably a condition called Fibroadenosis.

In the absence of detailed history I would like to know some details to help her more::
-Since when has she got the lump / pain ?
-Has the pain any relation to the menstrual cycle like increase just before the periods or so ?
-Any investigations done ? If yes, please provide the reports.
-Any specific reason that she is very small with no fat in the breasts- I mean is she having any other general medical condition like thyrotoxicosis or so.

I would advise the following in such a patient:
-Physical examination by a Surgeon.
-Ultrasonography
-Most important is FNAC.

This will help in getting the final diagnosis.
Further treatment will obviously be as per the final diagnosis, counselling with the patient and her relatives whether to go for medical management or surgery. IF surgery is opted for the possibility of further disfiguration of the breast, its acceptance and so on.

It just started this month simce her last period. She doesnt know if the lump is new or just the way her breast is made. She is just not well endowed. No reason for small breasts. She wants to wait until next period to see if paingoes away after that
