The pain you are feeling in the chest especially when drinking coffee and smoking might be due to possibly heart problems. If they did not check your heart at ER, I advise to consult with your doctor and ask to run following examinations :
- ECG
- stress test
- echocardiogram
If everything will be ok, then upper endoscopy is advised to rule out GERD / gastritis / ulcer.
For the moment, I advise to :
- avoid coffee, smoking and other foods that would trigger such pain
- have your meals regularly
- take OTC antacids after meals and ranitidine at bedtime
