Question: I m having pain in all my hand fingers and feet too....got all my tests done ...RA factor n crp..... But all tests r negative... My ESR z high at 70....wht z d cause for pain in hands n feet....????

Age is 42 , female ...symptoms since 3 months... No reddening of eyes.... Not able to understand about morning stiffness...but whole day pain z there... Even d finger tip of 2-3 fingers burn....pain z also in the backside of palm....no fever....no loosing of weight....eat v v well n 75 kgs wt....thyroid profile normal...hv low back since a month....yes have history of genital ulcers...no history psoriasis... Hv reflux easophagytis so lot of irritation in d throat bcz of acids reflux... I m mainly concerned about rheumatoid arthritis bcz it has no cure n it spoils one mentally.....i would avpid steroids at all cost ....

Reiter's disease or ankylosing spond

Detailed Answer:
Hi.
There is a possibility of Reuter's disease in where there is an association of inflammatory joint pain with genital ulcers.
Also back pain can be due to ankylosing spondylitis.
In my opinion you should get further investigated as your ESR is significantly high.
As far as Rheumatoid Arthritis is concerned, R-factor if negative doesn't rule out Rheumatoid Arthritis.
Get following investigations:
1. Anti CCP Antibody.
2. X ray hip with both sacroiliac joints.
3. Xray both hands.
4. HLA B27.
5. ANA
These investigations will confirm the above discussed diagnosis.
Steroids are not a modality that has to be surely used if you have Rheumatoid Arthritis. It all depends on the severity.
We need to first of all diagnose your condition.
Get investigated and write back to me with these reports.
