What Causes Pain In Hip When Suffering From Hip Dysplasia?

Question: Dr. XXXXXXX

Continuing our discussion about my 14 year old daughter's right hip... I visited an orthopedic surgeon (Dr. Burt - XXXXXXX NE) who diagnosed her with hip dysplasia. The measured angle of her hip was 21.3 degrees versus the normal 25-40 degrees. He is recommending PAO surgery and sent me to see Dr. XXXXXXX to see if she is a good candidate. Seeing as how this surgery is so invasive, I'd like to get as many opinions as possible about the diagnosis and recommendation for surgery. I could probably upload the Xray and MRI if it helps.
Thanks, XXXXXXX

My 14 year old daughter is having pain in her right hip. Lateral rotation is painful and restricted. Medial rotation and flexion are fine. Duration is 3 months. XRays are normal. MRI is normal, although orthopedic surgeon said he could not rule out labrum tear. Physical therapy did not help. Ibuprofen provides marginal relief. She is very active, primarily as a dancer (10 hours per week). MRI scheduled for next week, but I am pessimistic about finding an answer there based on what I've read through this site. Here primary care doctor is speculating piriformis syndrome, iliotibial tract tendonitis, or hip bursitis, but none of those are likely to be confirmed or rejected based on MRI if I understand correctly. Please help if you can, dance competition season is fast approaching!
Transient synovitis may be there.

Detailed Answer:
Hi,
Thanks for your query.

I'm Dr. XXXXXXX XXXXXXX

After going through your query I concluded that your daughter may be having synovitis of hip as she has restriction of movements of hip and x ray is normal(excludes arthritis). Now there are three possibilities
1.Transient synovitis- but age and duration is not favoring.
2. Perthes Disease(early stage )
3.Tubercular synovitis -but there is no other symptoms such as loss of weight , loss of appetite and low grade fever.

Till MRI report comes treatment is rest to joint , avoid walking and analgesic such as ibuprofen for pain relief. ESR , TLC and DLC test may be of help.
You can discuss with your treating Doctor about it.

Hope I have answered your query.
If you have any further questions I will be happy to help.

Detailed Answer:
Hi XXXXXXX
Thanks for your query.

After going through your query and previous queries I came to know that your daughter diagnosed with hip dysplasia. But it is mild and according to some if center edge(CE) angle is less than 20 degrees than it indicates dysplagia( your daughter has angle 21.3) .Normally angle is 25 degrees or more.So your daughter falls in uncertain group. In these cases muscle strengthening exercises may suffice. However decision depends on her symptoms and surgeons assessment.

I can better help you better if you upload x ray and MRI .

You can discuss with your treating Doctor about it.

Hope I have answered your query.
If you have any further query, I would be glad to help you.
