default
Posted on Thu, 11 Sep 2014
Question: Hi,

I'm a 23 year old male working in IT industry. Non- Vegetarian food habits. I used to smoke 4 cigarettes a day till last month since 4 years. I have suffered from tonsillitis 3 times in last 10 months. No extraction of the tonsils has been performed.

Recently I'm experiencing a dull ache in the lower part of my right rib cage. Sometimes in the central left rib cage. The pain is not sharp not continuous. It does not radiate to the back. I also have a problem of continuous farting. I do not take any medicine daily.
Three times in last 2 month I have experienced excessive hunger which is unbearable.
Three times in last 2 month I have experienced excessive thirst and excessive urination. Once I did a RBS with a glucometer and BSL was 91 mg/dl.

Please Help.

Brief Answer:
Upper GI condition needs to be ruled out.

Detailed Answer:
Hi,

I am a physician associated with department of gastroenterology for few years. Since your question is unattended for a while I shall take this opportunity to answer it.

At the outset this pain and associated complaints don't seem to represent a serious problem. However it would be worthwhile to investigate the causes. The most probably cause is related to upper GI pathology - such as gastritis or gall stones. I would order for an ultrasound scan and upper GI endoscope during acute painful condition to rule them out.

Your blood sugars are perfectly well. And those random episodes of excessive hunger and thirst don't seem to be related or have any clinical significance.

Please consider visiting your physician/gastroenterologist the next time you get this pain. The aforementioned tests can be considered after physical examination.

Until you get the tests done avoid spices, restrict smoking and caffeinated beverages. Keep yourself active with some form of physical exercise, preferably aerobic exercise.

Hope this helps.
