What Causes Pain In Right Lower Abdomen And Fluid In Uterus?

Question: A fortnight ago I was rushed to ER with excruciating pain on my right lower abdomen area on suspicion of appendicitis. A CT scan however confirmed whilst that my appendix is jolly happy, I have “fluid in my uterus, in the endometrial cavity and free fluid in the pelvis”. My pelvis and vaginal ultrasound were both normal. Based on this they diagnosed me with Pelvic Inflammatory Disease and ran the STI test which came back negative (both for chlamydia, gonoriah and herpes). They did find “heavy growths of gardenella vaginalis” but I understood this is not STD. I was given the 2 antibiotics for the PID (as if I had STDs) and they did lessen my symptoms a bit, but the pain is still there. I have also almost completely lost my appetite, finding it really hard to prep and eat something, the most I can manage is usually some soup and crackers with cheese, apple, green salad. Feeling constantly thirsty– I wake up thirsty at night and have to have a sip of water. I understand some of this is attributed to the antibiotics but still a concern.
To top it all of, one and a half months ago my 2nd Pap smear came back positive as a “low grade squamous intraepithelial lesion” (effect of HPV) and I am scheduled for colposcopy (end of next month – July). I have not had children (I am 35) there is no history of gynaecological issues in my family.
Can someone –honestly- tell me what is going wrong? Could the 3 things (liquid in my uterus, PID and failed pap smear) be connected? Is this saying that there is “big trouble in my uterus” but only “mild “ in cervix from where the PAP smear is taken – should I start preparing for a hard hitting discussion with my gyno?

Hi XXXXX, I have gone through your question and understand your concerns. Pain in the right lower abdomen, fluid in the uterus and the pouch in pelvis are suggestive features of pelvic inflammatory disease. However, loss of apetite, changes of low grade squamous intraepithelial neoplasia can be related and can be due to cervical cancer. It is to be confirmed by colposcopy and directed biopsy, if required. Since you are scheduled for colposcopy, it is the correct line of management. You should get a biopsy of endocervical canal also to rule out any occult cervical cancer, which is not detected by pap smear or colposcopy.
If there is an occult cervical cancer, then also it leads to collection of fluid in the uterus.

Thanks for the honesty - may I ask - should I attempt to move the date of the colposcopy from the end of the month to the start of this month? Will take make any difference? Shall I still carry on with the 2 set of antibiotics even thought I am confirmed as negative for STDS. Is it safe for me to have intercourse -should the lust strike- or it should be completely off the list until the colposcopy results are back?Can I start with some moderate exercise (such as swimming) in the hope of working my appetiate back up or it is not advised at this stage? Thanks

Hi XXXXX, thanks for responding. Yes, it would be better if you shift your colposcopy date earlier as diagnosis will be made earlier and appropriate treatment can be done.
Antibiotics should be continued keeping in mind that it can be pelvic inflammatory disease, which needs a complete antibiotic course to treat.
Intercourse should be avoided till the colposcopy is done, as it may alter its results.
You can very well do exercise at this stage.
I suggest you not to worry till the correct diagnosis is made.

apologies but whilst I wait for my gyno appointment I have to ask a follow up question in regards to this matter. 2 days ago - whilst still on the antibiotics, my period arrived. The pain and cramping during the 1st day was so bad I could barely get out of bed to grab a painkiller (stupid me for not leaving it near the bed) but had hardly any bleeding on the pad, whilst plenty was coming out during the visits of the restroom. Same thing happened on day2 - hardly anything on pads, plenty in bathroom. Now day3 my period is as good as over. The blood flow I have seen is about 75% less of my usual one if not 80%. The horrible menstrual pain is gone but the mild pain remains - is this normal when I am 95% through with the antiboitics I was given for PID (2x400mgr metronide a day +1dose of azithormycin)?

Hi dear, the pain during the periods will slowly get relieved with time. The antibiotics will act to clear the infection, but the effect of infection ( inflammation) will take time to heal, nearly2-3 weeks after the antibiotic course is over.
