Question: Hello, I am having a sharp pain on my right side in my pelvic region and some abdominal cramping. The cramping is not constant. My bowel movements are of a normal color, except for some dark black spots and I have diarrhea almost daily.

Such pain can be caused by several issues including appendicitis, inguinal hernia, ovaries cyst or large intestine tumor.
As you are over the age of 50, have cramps and diarrhea, partial bowels obstruction due to colon cancer should be ruled out at first. You may also have bloating and weight loss with this issue. Blood in stool can be present.
I suggest you to do not wait any longer with it. Please consult your doctor as soon as possible. Abdominal X-ray, abdominal ultrasound, blood work, urine analysis should be done at first. Colonoscopy should be second.

Thank you for your prompt response. My appendix were removed many years as was the 1 ovary that I had. What is an inguinal hernia? I am also having back pain just below my ribs on my right side. I just recently changed Dr.'s and am not quite comfortable with him, plus the fact that I have been to him 2 times in 1 month. I don't want him to think I'm a hypocondriac but I do need to see a Dr. I'm not sure if I should go back to him yet again, go to Urgent Care or the ER?

Inguinal hernia is a kind of protrusion of abdominal contents through the inguinal canal. In simple words intestines or fat from abdomen can get through the mentioned canal and appear as a lump under the skin. It gives lower one side pain along with lump. Such lump should be visible when standing and coughing and invisible/less visible when lying down. Pain when lifting heavy objects or when coughing is quite characteristic.
Back pain on the same side can point to urinary tract infection in which the right kidney is affected. With this condition you should have frequent urinating or burning while urinating. Such infection is quite frequent with diarrhea so it does not rule large intestine problems out.
If the pain is kind of stable for many days and today it has not changed, this is not an urgent. However if the pain is getting worse day by day, this should be checked right away. Symptoms which should force you to visit ER are:
- high heart rate (higher than 100)
- dark urine/low amount of urine
- pale skin
- bloated abdomen tender when palpating it
- occasional gas/stool obstruction
