Rt shoulder blade and mid back when I cough.
Brief Answer:
seems muscular pain only.
Detailed Answer:
Hello dear, thanks for your question on HCM.
I want to ask you few questions first.
1. Are you smoker?
2. Since how many days you are having cough?
3. Do have other symptoms like low grade fever, chest pain, weakness etc.
Your symptoms are mostly suggestive of muscular pain. As coughing causes forceful contraction of back muscles and may give rise to pain.
Another possibility is pleurisy (inflammation of pleura).
For this chest x ray is needed.
So want to know answers of above mentioned questions first, before commenting further.
Hope I have solved your quey. I will be happy to help you further. Wish you good health. Thanks.

Brief Answer:
chest x ray and PFT are advisable.
Detailed Answer:
Hello dear,
You are smoker, so I need to know couple of more details
Do you feel shortness of breath on exertion?
Have you ever undergone PFT?
Since you are smoker suffering from chronic cough (smoker's cough) which is now hurting, infection and bronchitis are couple of other conditions that can mimic muscular aches. I will order for chest x ray and PFT (Pulmonary Function Test) to rule out infection and bronchitis.
Smokers are prone to lung infection as their lung defense is altered and weakened by smoking. So you may have chest infection which causes cough and pleurisy. Pleurisy causes shoulder pain and back pain. Another possibility is bronchitis. For this you need need PFT.
So consult pulmonologist and get done chest x ray and PFT. If both are normal than your pain must be of muscular only. And quit smoking as soon as possible. It is not a good habit.
Hope I have solved your query. If you are not having further queries, please close the conversation and rate the answer. Wish you good health. Thanks.

Brief Answer:
you may have early COPD.
Detailed Answer:
Hello dear,
Thanks for your reply.
Shortness of breath is one of the earliest symptom of COPD (Chronic Obstructive Pulmonary Disease). COPD has two variants chronic bronchitis and emphysema. So you may have chronic bronchitis mostly. As you have smokers cough and shortness of breath on exertion.
So get done PFT as soon as possible for the diagnosis of COPD. PFT will also give idea about the severity of the disease. And treatment is depend on severity. So PFT is must for you.
Also get done chest x ray.
Chronic bronchitis itself can cause pain on cough.
So consult pulmonologist and get done PFT and chest x ray.
And I advice you to enroll yourself first in smoking de addiction programme. And I prefer nicotine gums for my patients who wants to quit smoking. I have no experience with vaping.
But your pulmonologist will definitely help you in this matter.
So consult pulmonologist and discuss all these.
