Brief Answer:
Needs medicines to control bleeding.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
The pain that you have can be due to the stitches or the excessive bleeding that you are having. You can wait for 2-3 days and see if the bleeding subsides spontaneously. In case the bleeding persists then you need hemostatic agents like Tranexaemic acid and methergin prescribed from your gynecologist. You can start taking iron supplements till then to avoid iron deficiency.
The pain in the episiotomy stitches 10 days after the delivery can be due to an infection in the area. This needs a direct examination by a gynecologist to find out. You can do a hot water sitz bath twice a day to relieve any swelling present in the area. Keep the area dry and clean especially after using the loo.
You can squat while passing urine and pour warm water while urinating as this will prevent the wound from getting infected.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.

Brief Answer:
Yes. Pain reduces in 7-10 days.

Detailed Answer:
Hello.
Thanks for writing again.
The pain in the stitches should have reduced by now. Doing a hot fomentation with war water will also help.
If the bleeding is heavy with passage of clots and you need to change more than three pads per day then secondary PPH needs to be considered.
Hope my answer is helpful.

Yes clots are like lumps. In that case you are having a heavy bleeding. You can attend an ER to control the bleeding and for necessary management.
