Brief Answer:
need to take low power antiinflammatory analgesics

Detailed Answer:
Hi
Your situation is not unusual in that we do quite commonly see patients with pain persisting after Varicocoele surgery. This happens because the veins in the ends of the veins Which have been surgically dilated have pooled up and their back pressure causes the intense discomfort till the new veins develop as alternate paths for drainage. This usually happens in the first two to three months but if it takes longer then the pain does not subside. This is one of the reasons why we do not suggest surgery for patients with Varicocoele with preexisting pain because it usually is only going to get worse.
What you can do is make sure your acrotum is supported by a good underwear.
You can try using tablets like Pentoxifylline to improve the circulation but do not expect miracles. Tramadol and paracetamol combination is good for pain relief but if there is significant pain or swelling you can try guardedly an antiinflammatory drug like indomethacin.
Hope I have been of help
Regards.
