Question : sharp pain in main joint of big toe, radiates into foot toward ankle, some swelling. had dvt near ankle earlier this year. could this be another dvt?

It does not seem to be another DVT. Gout is the most probable reason. Can you please attach a photo of it? Do you have any calf pain or swelling?
