What Causes Pain In The Groin Area And Thighs In A Diabetic?

Question: I am a 75 yr old diabetic II female.I have had 5 years of Facet blocks, TF injections, & Rhizotomy.As time went on, the blocks did not work, I went to a neurological surgeon, had 3 MRI, lumbar spine, thoracic, & certical. My spinal column was closing off, I was having numbness & tingling in my buttocks and private area. Surgeon said I needed L1-L4 laminectomy, no fusion, no rods or screws. I had the surgery April 27, did well, post operative visit 1 month later. Sciatica pain completely gone, so surgery was a success. Was recovering well, and I was dismissed by my surgeon. 1 month later, had a flare up of bursis in my hip. My hip specialist did 2 x-rays, said hips fine, but did an injection in my left hip to reduce inflammation of the burses. No pain for 2 weeks, pain came back, but gradually subsided, started physical therapy. As 2 months went by, the hospital physical therapy department noticed my walking was getting worse, my primary physician noticed I was getting worse. They had no suggestions as did the physical therapist. I have continued to get worse, started having serious pain in my groin both left and right, and sudden severe pain in the front of my thighs down to the front of my knees. Pain shifts from right to left. I now use a cane. Have great difficulty getting around, up 1 step is hard even with a cane. No one knows what is going on. I went back to the hospital pain management clinic, had 1 lumbar transforaminal epidural injection. Lasted 1 week. Getting worse. Scheduled to have lumbar transforaminal epidural injection in 2 days. Doctor says if that doesn't work, he will do a nerve block. What do you think of a nerve block?
any ideas about all of this pain, never had this pain before surgery,, just sciatica from outer thighs down to ankles.

I read your query and understand your concerns.

Although it is never possible to diagnose medical conditions without proper examination but considering the fact that the hips are normal, laminectomy was a success and no similar issues in the past indicates possibility of diabetes related complications. One such complication is peripheral neuropathy. In this condition the peripheral nerves get affected because of ongoing diabetes. In addition in the initial stage there is shifting nature of pain like left to right or vice versa.

The second possibility is pressure over sacral nerves. It is ossible that after bursitis the nerve got pressured leading to pain in groin areas. However this needs nerve conduction study to confirm.

Thanks for your answer. I will bring up a few of your points at the Sparrow Hospital Pain Clinic. I will be having an injection tomorrow. What is the difference between a Nerve Block and a Rhizotomy? Would a Chiropractor be of any help?

Thanks for reverting back to me.

Rizotomy involves surgical procedure where the pain carrying fibres of nerve are severed permanently while the nerve block is temporary process where the pain signal is blocked and carrying fibres left intact.

Chiropractor have no role in any of these two procedures however he can be of immense help in diagnosis of condition.
