The intermittent lower abdominal pain radiating down to testicles and leg is most concerning for inguinal hernia. A hernia occurs due to outpouching of intestinal contents through a weakened abdominal wall.

Physical (Inguinal-genital) examination can confirm the diagnosis. Management is best with surgical correction. It is better to undergo appropriate management as soon as possible to avoid complications like strangulation, obstruction etc. So please be advised to follow up with your primary care provider for further evaluation and management.
