Question: Hi, my father is feeling a pain in the middle chest while he is trying to swallow food 1 day after angioplasty operation on LAD artery.
What is the reason behind this?

Dear sir
A gastro-esophageal reflux secondary to the effect of newer drugs may be one of the reasons for such a discomfort. Other reason may be related to the cardiac procedure itself (injury to blood vessels) but that would have much more symptoms like difficulty in breathing or a low blood pressure. Remote possibility is that it is a kind of angina which he is mis interpreting as problem in swallowing, This would again have more features and ECG changes.
The best judge is your cardiologist who have operated upon the patient. This complaint requires a clinical examination to find out a diagnosis and solution.

Follow up: Dr. Sukhvinder Singh (25 hours later)
He is taking PLAVIX 75mg, ECOSPRIN, SELOKEN-XL medications. Is it due to these drugs that some gastro intestinal bleeding is happening. PLAVIX and ECOSPRIN cannot be stopped and this way he is unable to eat properly. What is the remedy for such patients ?

Dear Sir
Yes, I am aware that these medications are given to all post-angioplasty patients and first two of them are essential. That's why I kept this possibility. It is not necessarily bleeding but simple reflux can cause the symptoms. moreover this is only a clinical possibility which needs to be confirmed by examination.
The drugs need not to be stopped altogether and additional medications (which decrease gastric acid production) can prevent such a reflux, if at all this is the cause of his symptoms.
If your cardiologist is not available, then the internist or physician will be able to throw more light on it after examination.

Dear Doctor,
Today is day 3 after operation and he could eat/swallow substantial food. Esophagus pain in swallowing is also subsiding. Will you view this as reflux situation coming to its logical end ?

Or do you want us to conduct tests to ascertain the condition ? Following your previous answer, I can confirm that while this pain was on on day 1 post operation ECG was done number of times and all was normal.

Historically it seems to be over. No, further testing is always better guided by a good clinical examination. Since he was in hospital and was examined several times, further evaluation should be done as per advise of treating doctor. Repeatedly normal ECG is reassuring.
