I understand your concern
You have develop pain in the scar tissue of partial fistulotomy.
I would suggest to consult your surgeon and undergo clinical rectal examination and MRI of affected area to find the cause.
1. Reopening of fistula is not common after three months of healing.
2. Next surgery will depend on the cause and its severity
3. Infection always does not cause fistula is back.
Pain could be due to nerve compression in scar tissue, shearing force, high pressure during defecation, walking & standing.
4. It could be due to just painful scar tissue.
It is best confirmed by clinical examination and MRI scan
