Question: the last few days I've been noticing a tightness in the scrotum and today it's with pain and tightness. now they are loose with lighter pain. I feel some discomfort in the lower pubic abdominal area too. I saw a doctor back in March and he said I had a Hernia but could it be something else?

Brief Answer:
Hernia, testicular torsion, see doctor urgently for reevaluation

Detailed Answer:
Hi and thanks for the query,

Lower abdominal pains and tightness in the scrotum is almost diagnostic of a hernia. IN this case an inguinoscrotal hernia, meaning it moves from the inguinal area to the scrotal area.

The main fear in this situation is that the hernia can strangulate. Due to a compression of a particular area of the hernial sac, blood might not flow well there causing pain, and in extremes, necrosis (death) of the tissue around with serious consequences. the fact that you have started experiencing pain compels a reevaluation by your doctor and subsequent surgery.

Other conditions that can cause similar pains include torsion of specific masses around your testes, torsion of the testes around itself that usually causes very serious pain. However in this case, symptoms like pain of acute onset and vomiting (nausea) are usually seen.

It looks more likely to be a hernia evolving towards strangulation than a testicular torsion. I recommend you visit your emergency physician or family doctor for appropriate reevaluation and management.

Kind regards as I wish you the best of health.

when I meant tight is similar to how the testicles are during ejaculation. The pain around the testicles is more like a dull discomfort pain (similar to when somebody gets "blue balls") rather a sharp pain. Not sure if working out the day previous caused symptoms of hernia damage or if it's something else. Also I felt around the area and haven't notice any obvious lumps or anything of that matter.

I meant to say rather than a sharp pain.

Brief Answer:
Probably a hernia.

Detailed Answer:
Hi and thanks for the update,

Your description makes the diagnosis of a hernia most probable in this case. Other conditions that cause pain around the testicle are often associating with acute and very sharp pains. Absence of this type of pain and nausea and/or vomiting make hernia most likely in this case.

Kind regards as I wish you the best of health.
