Brief Answer:
get done repeat CT Coronary Angiography.

Detailed Answer:
Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
Since, your pain is persistent even after physiotherapy and muscle relaxant and other supportive drugs, so I advice you to get done repeat CT coronary angiography to rule out cardiac cause.
Your report is of 1 year old. And in that nonsignificant narrowing is present in RCA.
So possibility of progression of this blockage over 1 year is there. And you are not improved with other routine drugs. So we need to rule out cardiac cause in your case.
So my advice to you is , get done either CT coronary angiography or percutaneous coronary angiography. It will help you in rule out cardiac cause.
Hope I have solved your query.
I will be happy to help you further.
Wish you good health.
Thanks.

Brief Answer:
rule out cardiac cause first.

Detailed Answer:
Hello dear, thanks for your follow up question.
I have seen your MRI report. Thanks for the information.
I still advice you the same thing. Rule out cardiac cause first, by either CT coronary angiography or percutaneous coronary angiography. My reason is that you are not improving with physiotherapy, muscle relaxant and other drugs (used for neuritic pain). And your MRI cervical spine is not that bad. For your MRI abnormalities, the treatment you are taking since 7-10 days is perfect and you must be improved by these if spine is the cause. But you are not improving, so we need to think other causes.
Hope I have solved your query.
I will be happy to help you further.
Wish you good health.
Thanks.
