Since your cardiac work up is normal, I will suggest you to get chest x ray done. Since you are smoker there are high chances of bronchitis. An x ray will also helpful to rule out infection in lung which is very common in smokers.
And I also want to know your weight. If you are obese than you may need to loose some weight as your symptoms of headache, tiredness, day time sleepiness etc favors early symptoms of obesity related sleep disorder.
For your chest pain I think you should consult pulmonologist and rule out bronchitis first. If all these is normal than no need to worry it must be musculoskeletal pain. Take good painkillers. I still insist on quitting smoking immediately.

Now for your bronchitis part, I suggest you to first consult pulmonologist before going for x ray. My concern for this is because of two reasons.
1. You are smoker and chances of early chronic bronchitis is there. So better to confirm it with PFT (Pulmonary Function Test). This will tell you the severity also.
2. You have already diagnosed and treated for acute bronchitis but still not relieved, this demands expert consultation and may need of inhaled medicines.
So you can delay chest x ray and first consult pulmonologist.
