Brief Answer:
X-ray chest.
Detailed Answer:
Hi.
Thanks for your query.
Your history is more suggestive of the problem in the lower chest, particularly in the pleural space and/or the lower lung. Ultrasonography is showing normal gall bladder.
This is the reason you are getting excessive pain when you lay down or breath.
I would advise you to go for X-ray of the chest.
Also get your blood tested for evidence of infection.
Do you have any other symptoms like fever, chills, coughing ?

Brief Answer:
Costo-Chondritis.
Detailed Answer:
Thanks for your feedback.
Good to know about negative history.
The history of tenderness to touch under right breast is suggestive of a disorder called Costo-chondritis. In this there is inflammation of the lower ribs, where the pain increases movements, deep breathing and on touch.
This condition needs to be monitored as you take pain killers. I suggest you consult a local physician. He/she will know about this condition and can monitor the progress.
