The facts submitted by You are incomplete and I would need more information,before any opinion is given on the above health issues .
Hence I would suggest you to have Second Opinion on following health issues-

a-Consult with Surgeon-to fix if large left breast is causing any puffy looking at the armpit level.To fix this-get Mammogram and USG of left axillary tail of the breast,which would confirm the dilemma about the armpit swelling.

b-Urologist to resolve the dilemma about the doubtful Urine analysis.
Get physical examination done and CT abdomen,which would fix doubts about the presence of Single large,causing pain in left under rib intermittent pains.

c-Also I would suggest-to get Second opinion from-Orthopedic Surgeon who would physically examination and would fix Left Shoulder Bursitis-causing swelling.Get X-ray of left shoulder and USG of -left Shoulder,which would confirm if any bursitis is there or not.

In the given situation the above plan is the best optional plan,would resolve your above 3 health worries as discussed above.
