What Causes Painful Bowel Movements?

Question: I'm currently 21 years old and have had painful bowel movements accompanied by bleeding intermittently since I was about 10 years old. I tend to go through a couple days or weeks where this happens regularly and then can go either a few or many months without cause for concern. When I saw a specialist years ago he diagnosed me with what he believed was an anal fissure but said he was unsure. At this point, and with this persisting for so long, I've been using google to search out possible causes. "Piles" seems to be the most likely cause but at this point I'm beginning to fear waiting to go back to see a specialist again because I've seen this could also be a symptom of some forms of colorectal cancer.

Dear Mr Vaner

First and foremost let me reassure you it is highly unlikely to be due to colorectal cancer, which usually occurs in individual above 40 years of age.

Your symptoms: pain while passing stools and associated bleeding suggest anal fissure as the likely cause. Underlying constipation could be the cause of anal fissure. Piles or hemorrhoid usually cause painless bleeding which is much more in quantity.

Following are the predisposing factors for anal fissure

1. Decreased consumption of water
2. Decreased intake of fiber in diet
3. Stress

Exact diagnosis will be known by thorough clinical examination. I would suggest you to consult your Specialist or Surgeon for further assessment.

In the meantime I will suggest you the following

1. Ensure good intake of water: 2 to 3 liter a day

2. High fiber diet: 2 servings of vegetable salad and 2 servings of fruits especially citrus fruit will ensure adequate intake of fiber.

3. Oral laxative

4. Hot Seitz bath: To a tub of lukewarm water add 2 spoons of antiseptic solution. Sit in it for 15 to 20 minutes each session twice a day. It will help reduce pain and associated spasm
