1. This transient, recurring swelling seems to be related to the parotid salivary gland.
2. Get a neck ultrasound (USG) done when the swelling is present.
3. If it is confirmeed as a salivary gland, get a sialogram or scanning done.
4. The parotid salivary gland usually develops fine sand like stone debris which does not usually show up on an X-ray (unlike larger submandibular salivary gland stones). Chroniuc sialadenitis or strictures of the Stensen's Duct may also be the cause.
5. Salivary gland endoscopy is now possible to alleviate some of the symptoms if medical management fails. Medical management including a course of Clindamycin (which achieves high levels in saliva), anti-inflammatory medications, local massage and fomentation should be done. Blood tests to rule out Sjogren's Syndrome should be done.
