no lump in breast and not sure about engorge as she is only complaining that pain is only in nipple.
milk production is less and baby is noy able to breast feed. only option is to express milk pumping

These symptoms are due to infection of breast called mastitis common with breast feeding female.
You can give her pain killer and do some hot fomentation.
Keep watch on any lump and abnormal discharge from nipples.
pain in nipples due to cracked in nipples, apply some vaselline after every breastfeeding, do not stop feeding.
If fever persist take antibiotics after doctor consultation.

thanks for ur advice. now temperature increases to 101.5 and gave P650. could you pls suggest any antibiotics for this?
