To recapitulate: Male/26 - very active on unprotected sex - burning while urinating 5 days - testicular muscle pull - muscle pain in lower abdomen - both sides - like someone hits testicles -
The Urine reports looks fine and the pain which you explained looks to be muscular.
The muscle of the abdomen might have gone into spasm, soreness or sprain due to excessive sexual activity. The muscle suspending the testicles, called cremaster muscles is an extension of these lower part of the abdominal muscles, hence gone into spasm and causing the partial testicular rotation causing severe pain.
Another reason can be epididymo-orchitis.
This can be confirmed by the following:
Clinical evaluation and examination.
Ultrasonography and color doppler examination of the scrotum and the abdomen. This helps a lot to have a proper diagnosis. Get this done ASAP.
Till then take bed-rest.
Apply warm compresses over the lower abdominal muscles.
Take an anti-inflammatory medication like Ibuprofen to ease the swelling, inflammation and thus the pain.
Scrotal support if the testicles are hanging.
Plenty of oral fluids to keep urine tract flushed.
Antibiotic cover as per the reports of culture and sensitivity if it comes positive.
