What Causes Pelvic Pain With Normal Colonoscopy And CT Scan Of Pelvis?

Question: I have suffered with chronic intermittent pelvic pain for almost 2 years. I have had a colonoscopy, CT scan of my pelvic and lower abdominal area, tests for infection, a round of antibiotics as prophylactic measure, evaluation for uterine fibroid (1 doctor said yes after reading the ultrasounds (transvaginal) - 2 said no fibroid),second saline ultrasound turned up nothing and test for ovarian cancer. All were negative. The pain is intermittent, most days a pain level of 1 or 2 on the scale of 10. I have tried NSAIDs to know avail. I am currently on Gabapentin, which has helped. However, as of late I am once again experiencing extreme bouts, with pain so bad I must leave work. My doctor has refused to provide anything stronger to deal with the really bad pain days. Today is about a 7 on a scale of 1 to 10.

With your history it is very clear that most of the causes of pelvic pain is already rule out in your case.
But if it is possible then go for one more test that is laparoscopy to rule out endometriosis and other diseases of uterus.
In your case it seems that pain is psychological because you have tried so many investigation and treatment and nothing is found out and no treatment is effective so far.
There are few suggestion for you-
1) You can try opiates analgesic after consulting your doctor.
2) Pelvic floor muscle exercise, deep breathing muscle exercise.
3) Neromuscular stimulation.
4) Avoid stress and long hour sitting.
5) maintain proper posture.
6) Counselling.
Please tell me whether you achieve your menopause or not, any accident in past?

I have not yet reached menopause. There is no correlation between my increased pain levels and my infrequent periods. I have not sustained injuries in any accident. Both of my children were born vaginally. I exercise regularly and do kegals daily. I am in therapy already for bi-polar disorder, which is well managed with mood stabilizers and anti- depressants. The intense attacks are unpredictable and do not correspond with periods of increased stress. I head up the tax and financial planning division in the wealth management division of a trust company so my very stressful time is obviously tax time. I have not noticed any increase in the number of acute pain incidents during that time. A uterine biopsy was negative, which I understand does not address endometriosis.

With your history it is quite clear that the pelvic pain is not due to any gynaecological problem.
Have you ever visited to gastroenterologist, urologist for your pain?
As you said that you are suffering from bipolar disorder and taking antidepressant one of the cause of your pain may be Fibromyalgia.
You should go to your doctor and see for trigger point of pain to rule out this disease.
The treatment of this disease is also antidepressant, cold and hot therapy and massage on trigger point.
Meditation, yoga, and exercise also help which you are already doing.
