Brief Answer:
Possible prostate issue

Detailed Answer:
Hi and welcome.
There are 3 possible causes of this. First, this is most commonly caused by prostate enlargement or prostate infection, second, urinary stones may lead to blood in urine and semen and third, STD should always be considered. So he should do std tests, urine analysis and prostate ultrasound and everything will be cleared out. Wish you good health. Regards
