If you have positive anti TPO antibodies, you should check the the TSH level every six months.
The symptoms you described are suggestive for under active thyroid or hypothyroidism.
So the moment to start treatment may have come but it should be confirmed by blood tests.
I would recommend to check; TSH, Ft4 and Ft3 to confirm hypothyroidism.
An ultrasound of the thyroid gland is needed also as you have a nodule.
The blood pressure is not related to hypothyroidism.
If the difference in both arms is greater than 20 mm/hg report it to your treating physician.
