What Causes Persistent Fever And Fatigue In A Child?

Question: My daughter is getting fever every 8th day since past 7 weeks. In the initial 2 weeks, fever only came once and went off with just one dose of crocin, then next two weeks fever came 2 times(half day) and we had to give 2 doses. Then 3 doses and last two weeks we had to give crocin 4 times, she had fever for 24 hours. But today she got fever 6 times, had fever for 1.5 days. So with every week it is getting worse. She gets very weak and lethargic in the day of fever but otherwise she is active.

Details of my daughter -
Age - 3 year 2 months
Weight - 11.8 kg
Height - 90 cms

Her weight is constant since past 1 year between 11.5 & 12. It went up to 12.1 kg but then she was hosptalised in march 2014 due to acute pharyngitis & wheezing. After hospitalisation it had come down to 10.8. Now again it is back to 11.8. Before this periodic fever also she used to fall sick very frequently atleast once a month.

On clinical checkup nothing was found by any doctor. We have shown to 3 doctors as of now. We have got all blood tests done thrice in march, then april and then in May but nothing came in those reports. Montuse came negative 2 times. Malaria came negative 3 times. We have got her following tests done
-Malaria parasite smear/RDT : Result- Negative
- WIDAL Result- Negative
- Complete urine examination - 3-4 pus cells
- C reactive protein 4.2
- Urine culture Result- Negative
- Liver function test
- Abdomen ultrasound - Mild hepatomegaly (12 cm) in liver
- complete blood picture
- ESR 26 ( In April blood test it was 34)


One doctor is saying its malaria and malaria tests are not accurate every time. Other doctor is saying its primary complex which is very difficult to diagnose in kids. One more doctor is asking us to go and meet oncologist.

As per your experience what could be the possible underlying issue with my daughter. I understand that with unsupported clinical tests you would not be able to say wth 100 % confidence but atleast what do you suspect?

Most probable cause could be malaria ( in XXXXXXX ).Many a times report come negative for malaria inspite of malarial parasite having present in blood.So empirically malaria treatment should be given.
Other possibilities which should be ruled out are
.deep access
.adenoids
.infective endocarditis
.connective tissue disorders like SLE
.malignancies
Blood culture,USG abdomen,2 D ECHO should be done

What should be the dosage of lariago given to her for malaria? Her weight is 11.8 Kg and age is 3 years and 2 months.
Doctor has advised 10 ml twice a day for 3 days. Is it correct?

I do not think the dose mentioned by you was adequate.
My dose and schedule is this
You can give as 12ml - 6ml - 6ml - 6ml ( 0-6hr-24hr-48hr) or as
12 ml-12ml-6ml ( 0 - 24hr - 48 hr ) in which 0 stands for the time on which first dose was given and rest of the doses are in hours calculated from first dose. I also suggest to ask your doctor if it fine with him.

She doesn't have any other symptoms for the diseases you have mentioned. Doctors have checked her clinically. Only symptom is that she gets fever.

Her abdomen USG is already done and report is attached. Please have a look

I saw USG report.it is normal.Differentials given by me are for fever without any other symptoms and persisting beyond 7 days.So if fever persists then you discuss with your doctor along these lines
