Brief Answer:
possible allergy or liver issues

Detailed Answer:
Hi and welcome.

In most cases this happens as a result of possible allergic reaction to certain food, clothes, chemicals, soap or even sun. If Benadryl helps then it indicates such scenario.
But also, it may be sign of elevated bilirubin levels which is seen in some liver disorders.
If there are skin changes such as redness, exfoliation or white patches then this may be dermatologic issues as seen in psoriasis or seboroic dermatitis.
LAstly, some medications and neurologic issues may manifest on this way but this is less common.

I suggest to do blood work which include bilirubin levels and eosinophil count for a starT. Try to identify and possible alergens or medications that you may be allergic to. Also, take vitamin D supplements and avoid unhealthy food, alcohol and smoking.

Wish you good health. Feel free to ask further questions. Regards
