I'm Dr Sudhir Kumar
I have gone through your query and understand your concern. It may be due to cervical spondylosis ( arthritis) as it is common in his age. He is already on pain management and analgesic. I think he should get benefitted by glucosamine and Mecobalamine. He can also get vitamin D level as vitamin D deficiency is important cause of cervical pain. If vitamin D level is found low that he can take vitamin D3 supplements.He should also consult neurologist to exclude migraine and vascular causes. He can get MRI brain and cervical spine to exclude chiari malformation. MRI and CT scan brain exclude other causes also.

As he has very low in testosterone. So testosterone is likely to be beneficial for him.He can similarly benefitted by anabolic steroids such as nandrolone. As you have observed tightness in his neck muscles. This can be due to tension headache so I think anxiolytic such as alprazolam can be beneficial.
