Brief Answer:
Could be due to Sperm Granuloma

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that your nephew has undergone Vasectomy and now having pain in testis .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) When was the vasectomy done ?
2) Does he have swelling in Spermatic cord or testicle.?
3) Was ultrasound Scanning of scrotum done very recently?.

Normally persistent pain in testicles after vasectomy is due to what is called as sperm Granuloma that occurs due to leakage of sperms and formation of granuloma at the site where Vas Difference is transected .However in majority of patients this gets resolved in 3-4 weeks after taking anti inflammatory medicines like Diclofenac
twice daily .

Please get his ultrasound Scanning of scrotum done to rule out sperm granuloma .

Taking steroid injection at the site of scarring may help to resolve the issue .

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.
