What Causes Persisting Inflammation Despite Treatment Of Crohn's Disease?

Question: Hello, I was diagnosed wtih Crohn's disease at age 58 following a colonoscopy, January 2014. I had a severe allergic reaction to the initially prescribed mesalamine, and have had good success moderating my digestive symptoms through diet modifications. However, blood work still shows a moderate level of inflammation. The drug therapies available next are much more intense regarding side effects. So, does Crohn's need to be treated for the underlying inflammation even if the patient is feeling well? I will have a follow-up colonoscopy in July. I am taking iron to help fight persistent anemia. I see a gastorenterologist who I like and respect, but he is always so busy that I leave the office with unanswered questions. Thank you for your insight, BSL

Although the patient is feeling well,the pharmacological treatment is necessary because blood work still shows a moderate level of inflammation.
Mesalamine and other similar drugs called Aminosalicylates are more usual maintenance treatment in patients with Crohn Disease.
When it is not possible to use Aminosalicylates, Corticosteroids are the most appropriate treatment. You should not be worried about bone loss and others side effects of Corticosteroids because the required dose is low, for example Prednisone 10 mg per day (In Crohn crisis prescribed Prednisone dose is even 50-70 mg per day).
