Atelectasis is collapse of the alveoli in the right middle lung. This is commonly seen in chronic smokers leading to COPD(chronic obstructive pulmonary disease), infections, malignancy. The atelectasis and the pulmonary nodule might be related but needs to be investigated. As you have taken treatment for infective pathology only and its not working.
I would advise you to go for Ultrasound or CT scan guided fine needle aspiration cytology to rule out any other causes like Tuberculosis or malignancy. The treatment would follow accordingly.

For TB test - doctor here in Canada ruled out to do Quantiferron test- you can guide should I ask them again?

my IgM is low for last 3-4 years, non compliance of CPAP for obs sleep apnea. does these reasons contribute for present conditions?

Hi, dear. You may have other pathology also. You may have bronchiectesis. Repeated infection is commonly related with bronchiectesis. You can go for pulmonary function test or saccherine test for that. For tuberculosis you can go with TB PCR test. As far as nodule is concern you should go for aspiration cytology. It is the investigation of choice for that. Consult your doctor and ask him for guided FNAC. Take treatment accordingly.
Your present symptoms is not related to sleep apnea. So don't worry for that.
