Regarding head pounding while sitting or standing there are a couple possibilities that might contribute to this. One possibility is elevated blood pressure. Classically when the blood pressure is high it can create a significant pounding sensation that can be very uncomfortable. I would suggest that you try to have your blood pressure checked when you were experiencing one of these episodes. If your blood pressure is indeed high then it might be reasonable to consider medication to bring your blood pressure under better control.
Regarding word finding difficulty this may be related to your prior dramatic accident especially if it included aTraumatic brain injury. This can sometimes lead to periods where it might be difficult to complete a thought or sentence or find a particular word. Usually with continued challenge and work on speech and discussion these problems become less and less of a disabling issue.
I would try checking your blood pressure on a regular basis to see if there is any suggestion that you have hypertension. I would also bring this to your primary care doctor for a focused consultation and physical examination to see if he has any additional recommendations for you. I also would want to make sure that you are consuming a very well-balanced diet with vitamins and minerals which can cause problems if someone is deficient in a particular vitamin or mineral particularly involving speech and neurologic function.
If your doctor is not able to find an obvious cause that he might consider sending you to a neurologist for evaluation.
